Satisfaction in almost every area of life (career, finances, relationships, etc.) depends on your physical and mental health. How satisfied are you with your health? Which areas contribute to your overall satisfaction? Which detract? How does it all balance? Use the health wheel to get a picture of your overall health and well-being.

5 Middle of the Road.
It's OK, but there are changes that you could make here.
1 Completely Unsatisfied.
You'd be much happier if everything about this category was completely different.
After you rate each category. Connect the dots. If the line were a wheel, how "Bumpy" would your life be? Focus on the uneven areas first, to achieve a more "balanced" health wheel.
Education. Consider how you keep yourself informed about current health issues.
Self Care. This includes taking care of personal hygiene and getting enough rest.
Leisure/Recreation. This includes enjoyable activities that energize you.
Healthy Rituals. This includes regular family meals, family entertainment, spiritual life and emotional well-being.
Nutrition. Consider meals, snacks and beverages. Planning and implementing meals.
Personal Growth. This could include intellectual and/or spiritual growth.
Maintenance/Prevention. What do you do to promote health and prevent illness. Consider doctor or dental appointments, self examinations.
